Hourly Rate: $18.15-$22.74

Duties & Responsibilities

    • The Foster Home placement coordinator will arrange for out of home placements for children coming into licensed care placements. This involves an in-depth knowledge of available resources as well as the child welfare system, and the ability to match a child's unique needs to the least restrictive setting available.
    • The Placement Coordinator will review placement intakes to assess child's unique needs, and to ensure the intake report accurately reflects the presenting issues of the child(ren) to determine appropriate available placement options in a least restrictive level of care.
    • Contact placement providers (i.e., foster homes, group homes, shelters, etc.) to discuss the child's needs and secure the most appropriate placement within two hours of receiving intake.
    • Ensure all information is gathered regarding medical information and follow the CMAT policy, ensuring all information is documented in the SACWIS system; complete all Match reports within 24 hours of placement.
    • Participate in daily morning meetings to assess the children in need of placement, analyze changing needs for children disrupting from current placements; complete and distribute the pending placement spreadsheet to the appropriate parties.
    • Complete and communicate the daily transportation and daycare needs to the appropriate parties ensuring that all children are accounted for.
    • Participate in the weekly clinical/ Placement meeting addressing the special needs of high-end placements and coordinating and planning for the child's step down into less restrictive settings; work collaboratively with Child Protective Investigators
    • Ability to work on call hours outside of arranged daily business hours to include nights and weekends, as designated by Supervisor.

Qualifications

  • Bachelor's degree, from an accredited College or University, required; degree in social work or related human services field, preferred.
  • One-year direct experience in the delivery of services to families and children with understanding/knowledge of child welfare services and requirements required.
  • Possess knowledge of at-risk adolescents, troubled youth and their families. Possess knowledge of 65C-28. Ability to work well with youth, families, staff and agencies, and to maintain professional working relationships. Ability to work independently and utilize initiative, ability to be organized and to meet deadlines. Must be proficient in specific technology or systems that are utilized to track or manage day to day activities as assigned.
  • Must be able to meet requirements for Eckerd's Auto Insurance and be able to drive for business purposes.
  • Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.

About Our Program:

Eckerd Connects' Foster Care is an approach that caters to the physical, emotional, and social needs of children and youth in a supportive family setting until the natural family can be reunited or a permanent placement through adoption can be arranged. All children we serve in foster care receive an individualized package of wrap around services that includes home based individual and family therapy, educational support and advocacy, linkage to psychiatric care when needed, and daily instruction in identified need areas of basic living and social skills to help the child gain the skills they were never taught before. Eckerd Connects provides a continuum of wrap-around services for the family from the first inquiry call until the child graduate's high school.
