Overview

Manager PACE Program

PACE (Program of All-Inclusive Care for the Elderly)

Monday-Friday 8:00AM - 5:00PM

Full Time Benefits

Under supervision by the PACE Executive Director, provides leadership to assure the quality of care and operational effectiveness in service delivery of PACE program. This position requires utilizing skills of assessing, planning, organizing, and evaluating the provisions of participant care and demonstrates system thinking, performance improvement to improve organizational processes. The individual serves as a positive role model and promotes employee professional development and satisfaction within the organization.


Responsibilities

* Maintains a working knowledge of applicable PACE Federal, State, and local laws and regulations, the Compliance Accountability Program, Code of Ethics, as well as other policies and procedures in order to assure adherence in a manner that reflects honest, ethical, and professional behavior.

* Maintains knowledge of PACE interdisciplinary team member standards of practice and assures that clinical delivery of services meets defined standards.

* Provides leadership in the development, implementation and evaluation of PACE service goals and outcomes.

* Assesses plans, organizes and evaluates the quality of participant care provided by the interdisciplinary team members and contracted service providers.

* Assists in the professional development of team members by identifying core competencies and assesses competencies based on joint visits, verification of skills, and areas for improved performance by an employee.

* Facilitates the development of strong, cohesive teams utilizing communication skills, team building, problem solving, and financial skills

* Evaluates and manages employee performance within PACE and UnityPoint Health guidelines. Evaluates employee performance within established time frames (30 day, 90 day, and annually) and more frequently as needed.

* Participates as a member of the PACE Leadership team in organization and program planning, goal setting, and evaluation.

* Assists in assuring efficient utilization of organizational resources through monitoring of financial reports, and other business reports. Identifies opportunities to maximize operational efficiencies.

* Facilitates monthly department meetings and individual employee rounding.

* Facilitates Quality Initiatives as assigned.

* Completes Human Resource and Employee Health functions for assigned staff, including scheduling, payroll approval and Human Resource personnel files.

* Ensures risk management and feedback resolution requirements are implemented according to PACE and UnityPoint Health guidelines.

* Maintains collaborative relationships with other community health care providers.

* Ensures a safe, functional, accessible and comfortable environment for the delivery of services to participants and staff.

* Participates as a member of the Administrative team on call rotation.


Qualifications

Education

Bachelor's degree required OR a combination of education and experience.

Experience

Minimum of one to two years of progressive management and leadership experience required

License(s)/Certification(s)

Valid driver's license when driving any vehicle for work-related reasons.
Valid Auto insurance
Maintain current Mandatory Reporter status

Knowledge/Skills/Abilities

Must possess interpersonal skills sufficient to interact effectively with PACE participants and their families, as well as interdisciplinary care providers, peers, and other health care professionals.
Ability to lead and work within the Interdisciplinary setting.
Able to manage changing priorities per needs of the program.
