The Nurse Practitioner demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems, prevention of illness and injury, maintenance of wellness, and provision of comfort. This position assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The Nurse Practitioner continues to perform many of the same interventions used in basic nursing practice. The difference in this practice relates to a greater depth and breadth of knowledge, a greater degree of synthesis of data, and complexity of skills and intervention. Physical Requirements (includes use of assistance devices as appropriate): The Home Based Primary Care group (HBPC) (formerly known as House Calls, est. 1984) consists of interdisciplinary teams (MD, NP, RN, SW, pharmacy support) that provide primary care for patients who find it challenging to leave the home to be seen in clinic. The HBPC team is composed of two teams, one North and one South, with an MD and a combination of APPs covering each team.
A key goal of the program is to provide services for the individual patient in their place of residence so that dignity is maintained, comfort and rehabilitation potential is maximized and to provide patients and their families information and education to assist them in adjusting to life-style changes that result from their disease or disability.
