Job Description

Plans, organizes and supervises outreach team and coordinates administrative support services for designated market. Promotes wellness and disease prevention through education, assessment, goal-setting, and special programming.

Job Responsibility

1.Provides strategic individual and group coaching aligned with clinical priorities.
2.Works with patients to develop strategic self-help plans and goals and ensures that patients adhere to their established plans.
3.Coaches patients through individualized detailed health plans by continuously supporting and encouraging healthy habits.
4.Reports on patient health status and progress.
5.Conducts wellness training courses.
6.Participates in community educational and outreach initiatives.
7.Ensures that wellness programs meet customer needs and satisfaction.
8.Often operates in ambiguous situations and provides process/technical/functional guidance to others.
9.Assignments are of significant impact to the organization.
10.Majority of contact is with stakeholders across the system as a subject matter expert and issue leader.

Job Qualification

*Bachelor's Degree required, or equivalent combination of education and related experience.
*10+ years of relevant experience and 2+ years of leadership / management experience, required.
*Specialized certifications as needed.
