Overview

The Community Health Worker (CHW) collaborates with clinical and medical staff to provide culturally sensitive one-on-one support and guidance to patients and families with social determinants of health (SDH) needs. This position requires an ability to uphold the principles of confidentiality, and implements and supports the philosophy, mission, values, standards, policies and procedures of Nationwide Children's Hospital.

Responsibilities

    Addresses identified patient and family SDH needs and improve patient/family access to community resources.
  • In collaboration with clinical staff, link patient/family to essential support services including housing, nutrition assistance, employment services, and other needs as identified by the patient/family and clinic team.
  • Assists in conducting follow-up calls to patients and families to confirm linkage with identified community resources and/or services, assess potential barriers, and aids in navigating resources and services. May accompany patients and families to social service appointments.
  • Provides appropriate documentation of patient and family interactions in EPIC.
  • Establishes and maintains close relationships with housing resources, food pantries, utility companies, government agencies, treatment facilities and other community-based organizations.
  • Assists NCH clinical staff, supervisor, and organizational partners in the decision-making process for improving linkage of patients/families with community resources.
  • Collects and report data on program activities.
  • Reviews and recommends potential resources that are culturally sensitive to the targeted community.
  • Remains informed of trends, developments, as well as state-of-the-art prevention efforts and strategies, and general information regarding the targeted community.


Qualifications

Education:

  • High School Graduate or equivalent required. Associates or Bachelor's degree preferred.

Certifications:

  • Community Health Worker certification by the Ohio Board of Nursing required or Completion of the Community Health Worker Training Program; certification must be obtained within 90 days of employment.

Skills:

  • Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.
  • Ability to work as a part of a team, with diverse populations, (i.e. racial/ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.
  • Strong attention to detail and organizational abilities required.
  • Sound decision making skills, with the ability to maintain highly confidential information. Moderate computer skills required (familiarity with Microsoft Word and Excel, preferable).
  • Ability to travel and work flexible hours, including some evenings and weekends.

Experience:

  • 1-3 years of professional experience with a social service agency, community-based program, or health-related institution.
  • Experience working and/or volunteering with children and/or families required.

MINIMUM PHYSICAL REQUIREMENTS

  • Sitting for extended periods - Constantly
  • Talking on the phone/in person - Constantly
  • Typing on a computer keyboard - Constantly
  • Standing/walking Occasionally
