Job Description:

This role supports Mother Baby at United Hospital.

Provides assessment, triaging, treatment planning, documentation and a variety of healthcare services to patients with comprehensive care needs, in a specialty population. Collaborates with multidisciplinary professionals to integrate nursing interventions into a comprehensive plan of care to enhance patient outcome. Specializes in a delineated area of practice with evidence-based competencies associated with that specialty. Partners with unit leaders to educate staff and other health providers to enhance patient outcomes. Services as a liaison between other departments, clinics and applicable community resources.

Allina Health has a robust Mental Health and Addiction Clinical Service Line that offers the full continuum of care including integration, specialty outpatient clinics, addiction and co-occurring services, partial hospitalization and day treatment, and inpatient care. There are more than 290 mental health and addiction clinicians dedicated to the highest quality patient care across 45 primary care clinics, 6 integrated psychiatry locations, 4 hospital-based clinics, 15 partial and day treatment programs, 15 Allina and affiliated Emergency Departments, and 7 hospitals with 270 inpatient beds. The Mental Health and Addiction Clinical Service Line is led by both psychiatry and psychology clinical leadership, in partnership with operations to advance mental health care for patients across Allina Health.

Principle Responsibilities

  • Participates in Patient/Client care
    • Conducts comprehensive, holistic wellness and illness assessments of individual or populations using evidence-based techniques, tools, and methods.
    • Synthesizes assessment data, draws conclusion about individual or aggregate patient problems related to nursing interventions.
    • Identifies expected outcomes of interventions and anticipates ethical conflicts, that may arise and plan for resolution.
    • Formulations appropriate differential diagnosis, with plan of care to address complex, acute and chronic health care needs.
    • Develops and implements evidence-based nursing interventions.
    • Collaborates with multidisciplinary professionals to integrate nursing interventions into a comprehensive plan of care to enhance patient outcomes.
    • Facilitates patient transition between and within health care settings including discharge plans, patient follow-up in collaboration with the interdisciplinary team.
    • Evaluates effects of nursing interventions for clinical effectiveness, patient responses, cost effectiveness and ethical considerations.
  • Evaluates Nursing Practice
    • Assesses patterns of outcomes related to nursing practice, drawing conclusions that may require change, maintenance or enhancement.
    • Anchors nursing practice to evidence-based information to achieve outcomes.
    • Mentors nurses and works collaborative with nurses to develop new skills and apply research to practice with educational programs.
    • Evaluates the effect of change on clinical outcomes, nursing and other personnel satisfaction, and the collaboration with other disciplines.
  • Assesses and plans for Organization/System issues
    • Uses/designs system-level assessment methods and instruments to identify organization structures, functions and system-level variables that impact nursing practice and nurse-sensitive patient care outcomes.
    • Develops innovative solutions to nursing practice issues, leading nursing and multidisciplinary groups in implementing patient care programs.
    • Contributes to development of multidisciplinary standards of practice and evidence-based guidelines.
    • Selects evaluation methods and instruments to identify system-level outcomes of programs of care.
    • Documents and disseminates outcomes of system-wide changes, impact of nursing practice, and Clinical Nurse Specialist work to stakeholders.
  • Other duties as assigned.

Required Qualifications

  • 2 to 5 years in a specialty area

Preferred Qualifications

  • Master's degree as a clinical nurse specialist (CNS) from accredited program
  • Advanced Practice Nursing degree (MNP or DNP) accepted in lieu of CNS program

Licenses/Certifications

  • Licensed Registered Nurse - MN Board of Nursing required upon hire
  • BLS Tier 1 - Basic Life Support - Multisource required by completion of orientation
  • Advanced practice certification in specialty APRN through MN Board of Nursing required upon hire

Physical Demands

  • Medium Work*: Lifting weight up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), Up to 25 lbs. frequently
