Overview

The Clinical Social Worker is responsible for providing developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. This Program Coordinator functions as a member of the Social Work department in collaboration with legal risk management, interdisciplinary teams to follow up with Mandated Child Abuse & Neglect Reporting of staff in order to monitor intake dispositions and provide feedback to Mandated Reporters so that interdisciplinary teams may determine additional treatment planning, advocacy, and support to families while addressing situations which are stressing families and improving potential for good outcomes for vulnerable patients. The program coordinator supports patients and their families as needed to improve or maintain social, emotional, functional and physical health; provides consultation, education, and training to staff regarding psychosocial issues & mandated reporting that impact the delivery of optimal health care services; and participates in the development and delivery of education, research and continuous quality improvement. The program coordinator is responsible for the overall coordination and implementation of a program targeting a specific population and manages pertinent expenses, data collection, and stakeholder relationships to accomplish program goals and objectives.

Demonstrates competencies defined (*) in Social Work Program Coordinator responsibilities in providing age-appropriate care to patients from the age of birth to 21 years, and some select adult patients.

Responsibilities

1. The Program Coordinator implements and supports the philosophy, mission, values, standards, policies and procedures of Nationwide Children's Hospital and the Patient Care Services Division.

2. Collaborates with Clinical Social Work leadership and legal risk management to plan and develop program goals and services, protocols, and best practice guidelines and measures and evaluates program outcomes.

3. Maintains clinical expertise in his/her profession; fills clinical role as needed and serves as resource and consultant to team and other staff regarding clinical challenges and dilemmas.

4. Provides oversight of Clinical Medical Social Work services to children, adolescents, and select adult clients in program populations by assuring appropriate assessments, managing care and treatment, making referrals, and advocating for services.

5. Collaborates with legal risk management and interdisciplinary team to identify, assess, and report suspected child abuse as required by law, with particular attention to risk reduction and prevention.

6. Develops documentation practices that support a work queue that can be monitored, addressed, and communicated back to teams.

7. Develops Reporting for the organization that helps to understand the responses by Child Protection agencies to Mandated Reporting and the ability of multidisciplinary teams to respond sooner dependent upon the Intake and Investigation dispositions.

8. Documents clinical practice in accordance with regulatory agencies, Social Work Department, funding agency requirements, and other professional standards.

9. Participates in ongoing education related to information technology requirements for the department. Demonstrates competency in PC and mainframe computer applications and software needed to perform job functions.

10. Supports advancement of social work practice through one or more of the following:

a. Participates as a speaker/presenter at professional conference

b. Participates as a speaker/presenter to community groups

c. Promotes staff involvement with professional organizations and activities

d. Participates on task forces, committees, or advisory groups (department, legal risk management hospital, community)

e. Participates on appropriate Quality Teams

f. Participates in scholarly activities

11. Identifies opportunities for and provides professional consultation to staff and community professionals regarding psychosocial issues related to children's health and child protection issues.

12. Contributes to or presents professional educational programs and in-services regarding psychosocial issues in area of specialty that enhance care and services for children. Coordinates, facilitates, and/or presents at various professional and/or patient educational seminars and retreats.

13. Participates in the development of clinical practice expectations and clinical/social/financial outcome indicators for given population. Analyzes patient and aggregate variances and recommends changes for more effective, efficient, and collaborative delivery systems.

14. Oversees implementation, scheduling, and staffing of program services (e.g. orientation, team meetings, patient education, clinical services, care coordination, conferences, and clinics), and supervision of any program staff.

15. Contributes to staff performance evaluations related to the scope of responsibility.

16. Monitors program budget and ensures adherence to budget guidelines in the implementation of program activities. Reviews new grant instructions, establishes deadlines, prepares and writes grant application proposals with Project Director. Maintains contact with grants administration to complete budget reports for funding agencies.

17. Develops and maintains relationships with appropriate community organizations. Participates and/or provides input to appropriate committees and planning bodies, both internal and external, and encourages participation of staff in these department, hospital and community groups as appropriate.

18. Prepares interim and final reports as required by the organization and/or funding sources.

19. Participates in quality assurance and productivity standards including time and accurate record keeping. With Clinical Medical Social Work leadership and Legal Services, plans and coordinates activities to meet regulatory standards such as The Joint Commission and to address specific quality issues such as incident report follow-up.

20. Performs other related duties as assigned.



Qualifications

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

1. Current LISW in Ohio required. LISW-S preferred.

2. Master's Degree in social work required.

3. Two years of experience with child protection cases in pediatric health setting required.

4. Strong organizational and interpersonal skills (detail oriented).

5. Ability to function independently and as part of team, with demonstrated ability to coordinate daily activities and problem solve in innovative and flexible ways.

6. Understanding of the Advanced Generalist Social Work practice in the pediatric healthcare setting.

7. Ability to work effectively and collaboratively with diverse, multi-disciplinary groups and stakeholders.

8. Strong verbal and written communication skills.

9. Must be enterprising, diplomatic, and proactive at solving problems.

10. Ability to manage and meet deadlines.

11. Ability to develop clear outcome goals, track and manage data, and have proven ability to improve program performance to achieve goals.

12. Research and analytic skills necessary for information gathering and synthesis.

13. Ability to interface with all levels of internal and external individuals, departments and organizations.

14. Ability to develop community and fundraising partnerships and write grants to assist in raising funds to sustain/grow program.

15. Proficient in the use of Microsoft Office Suite, data base management, desktop publishing, graphic packages, and survey tools.

MINIMUM PHYSICAL REQUIREMENTS Must be able to see, hear, speak, and perform manual tasks with or without accommodation and care for oneself, with little or no difficulty. Performance of principal duties and responsibilities may require ability to travel.
